Awana Sign Up 2021-2022

Please fill out this form for each child who would like to participate in Awana this fall. We will begin September 8, 2021 and will have all clubs operational for children ages 3 through 6th grade. While you need to fill out a separate form for each child in Awana, you only need to add other person(s) authorized to pick up your child once. Once you submit the form, you will be contacted by the Awana leadership for payment.
